Waiver and Liability Release Form
This is a general Release and Waiver form agreed to by each participant in classes operated by Shannon Burton, LLC. By reserving a class, participants are releasing Shannon Burton, LLC and any partners and site location owners from liability incurred in the participation in the activities listed below.
Participants declare that they are in good physical and mental health for participation in all activities selected. They understand that physical activity and emotional vulnerability may be required and that they have no known disabilities or health problems that will present any risk to participation in these activities. Participants agree not to participate unless they are medically able and assume the risks of the activities listed below.
In consideration for the ability to participate in these activities, participants waive and release Shannon Burton, LLC, its partners, and all of its agents, representatives, and successors from all claims and liabilities of any kind arising out of their participation in these activities.
Participants agree to participate in their choice of the following activities under the observation of Shannon Burton, LLC including, but not limited to verbal coaching, massage, pleasure mapping, arousal exercises, touch-based exercises, role-playing, impact play, bondage, and sexual exploration. Participants understand that risks include, but are not limited to, superficial bruising, short-term muscle soreness, exacerbation of undiscovered injury, and emotional discomfort.
Participants acknowledge that:
- it is their responsibility to inform Shannon Burton of any discomfort they feel, be it physical, mental, or emotional, during their class so that instruction can be adjusted accordingly
- participants or the instructor may terminate the class at any time
- they have been given a chance to ask questions about the class and their questions have been answered
Sex coaching and classes assist participants with matters such as sexual intimacy, personal goals, and relationships. A coach is in partnership with the participant to improve their personal life. The focus is to help the participant consciously create a vision and goals for life. Classes helps participants think differently and learn what’s best for their personal growth. Coaching is not therapy nor counseling.
These services do not include the practice of medicine or psychology or counseling. Participants agree that they are fully responsible for their physical, mental and emotional well-being, including their choices and decisions while engaging in these services. If they are under the care of a mental health professional, they will make the mental health professional aware of their sessions with Shannon Burton. If any type of counseling or mental health treatment is needed, the participant is to be referred to individuals who provide that care.
Participants understand that identifying personal information will be held as confidential except as required by law.
